This form will generate a chat notes document for you based on the information you enter on the following pages. The result generate the PDF for you to download and send to your fellow co-hosts. There are four main parts: Introduction followed by... PART 1: #90SecondQuestions. PART 2: Diabetes Diagnosis & Journey or Diabetes Connection and career background. PART 3: Main Topic of the #diabeteschat. PART 4: Mental Health, Advocacy and Peer Support. Any questions that you feel are not required you can delete them and this will exclude it off the PDF. The Guest Speaker Chats start at 8:30pm and finish around 9:30pm. The hosting team welcome questions and comments to be tweeted using the hash-tag #diabeteschat and also people can request the microphone to speak.

PART 2: Diabetes Diagnosis & Journey

Diagnosis - Please share with the #diabeteschat community your diagnosis with diabetes; when were you diagnosed and how old you were. Follow up questions to ask below:
- What symptoms did you have when you were diagnosed with diabetes? - Have you got any early diagnosis stories to share? - How long did you spend at hospital when diagnosed? - How did you/your parents feel at the time of diagnosis? How was this for you & your family at a challenging time? - Did you know much about diabetes or have anyone close (family/friends) who had diabetes or was it learning from scratch?
Journey - How was your diabetes journey progressed over the years and what do you use now? Please share your journey with us. Follow up questions to ask below:
- How do you administer insulin (syringe, injections, pumps); now and your regime over the years since being diagnosed? - How do you monitor your blood levels (finger pricking, Flash, CGM)? - How does it help your everyday life and what are your pros/cons of having and using technology to manage your diabetes? What are your experiences of using technology - how has it impacted on your quality of life? What are your hopes for the future for Diabetes Technology? - Is there any memorable moments that you take away from your early days living with diabetes - like your first HYPO, injecting for the first time, etc… - What have been the ups and down moments and what challenges have occurred along the way? How do you deal with these challenges? - What do you wish you'd known when you were newly diagnosed? - Type 1 diabetes is commonly classed as a physical condition, but we all know there is an emotional burden attached to it. Could you describe any experiences of diabetes stress/burnout or your thoughts on this.
